Calcium chloride bite me after 10 years of using it wrong
I was pouring a slab for a carport in Phoenix back in July and the mix was flash setting on me before I could even bull float it. Turns out I been adding calcium chloride straight to the drum instead of mixing it with the water first, which caused hot spots and crusting. Had to tear out 400 square feet and redo it the next morning with a bottle of water sitting in the shade. Anyone else ever toasted a pour with an accelerator mishap, or is it just my dumb luck?

Man that's brutal, but honestly I gotta ask, how the hell did you not catch that sooner? Did the bag not have directions on it or were you just winging it from habit? Cause ten years is a long time to be doing it wrong without somebody on site calling it out.
